Baseball/Softball Forms

The forms listed on this page are the most current, and the only versions acceptable for use.

Please discard any older versions of these forms that you have.

Please pay close attention to the form you are clicking.

For regular season, only fill out forms in the “Regular Season” section.

For All-Stars/Tournaments, only fill out forms in the “All-Star/Tournament” section.

In any form, place the cursor over a data box (blank line where you enter information) and a brief description of what information goes in that particular line will pop up.

After you have opened a form, you can fill out the required information and save the form on your computer.

Please pay attention to the name you give the form and the folder you save the form to.

After you have filled out the form, print the form and mail to the appropriate director in your State, by the designated deadline.

If your State allows it, you will be able to attach the form to an email (contact the appropriate director in your State for special instructions and/or recommended procedures).

This form starts the process for you to be able to play Dizzy Dean Baseball.

This form is for listing the number of teams in an age group and for listing the contact information for your park.

Payment is required with this form.

TEAM CERTIFICATE OF ENTRY

This form is for listing every team you are applying for sanction on and for listing every player on the team along with required contact information.

ALL-STARS/TOURNAMENT FORMS

These forms are required for you to play Dizzy Dean Baseball/Softball All-Stars

TOURNAMENT TEAM ROSTER

This form is required for every team participating in All-Stars/Tournament play.

This form is for listing the team name, players' names (along with required information), and coaches names and information.

This form will be checked at your credential meeting and is required to be available at every all-star/tournament game.

SHARED FORMS FOR BASEBALL AND SOFTBALL ALL-STARS/TOURNAMENT PLAY

SAFE SPORT FRANCHISE FORM

This form must be submitted with the Application For Sanction Form.

CONSENT FOR TREATMENT

This form is for listing a player's name, medical information, and contact information.

This form is required for every player who is participating in all-stars/tournament play (each player must have this form completely filled out and this form must be available for inspection at all times).

WORLD SERIES ENTRY FORM

This form is required for any team wishing to participate in the Dizzy Dean World Series. This form must be properly filled out and submitted to the State or National Director for your State.

SCHOLARSHIP APPLICATION

This form must be used if you are applying for a Dizzy Dean scholarship.

A player must have played at least 4 years on a sanctioned Dizzy Dean team for the player to be eligible for a scholarship.

There are two pages in this form. The first page is the contact listing (to assist in finding the proper person to contact). The second page is the actual application.
